Heart Failure

Appropriate Care Score: Also known as the perfect process score, it’s the percent of patients that received all the recommended and needed care that is monitored by the Quality Department. The target score is 100%.

Being Readmitted within 30 Days Observed/Expected Ratio (O/E Ratio): A ratio of less than 1.00 is better than expected. When the result is less than 1.00, patients were able to stay out of the hospital more often than would be expected based on the type of illnesses they have. (Note: A ratio lower than 1.00 is good.)

Mortality Observed/Expected Ratio (O/E Ratio): A ratio of less than 1.00 is better than expected. When the result is less than 1.00, patients did not die even when they were expected to due to their illnesses. (Note: A ratio lower than 1.00 is good.)
