Diabetes Self-Management Program

Our program offers a variety of services to assist you in managing your diabetes including:

  • Convenient group classes, led by experienced diabetes professionals held weekly in the morning, afternoon or evening.
  • Classes with a registered nurse to learn or review self-care skills, such as blood sugar monitoring, preventing and treating complications and medication managements.
  • Classes with a registered dietitian for help with meal planning, grocery shopping and eating out.

Recognized by the American Diabetes Association, this self-management program lasts for 10 hours and is presented during three classes. The classes cover the 10 core content areas required by the American Diabetes Association and are taught by certified diabetes educators. Upon completion of the initial program, follow-up sessions are available.

Anyone with a doctor’s referral is welcome to participate in the Diabetes Self-Management Program. Whether you want to better control your blood sugars, eat a healthier diet, avoid complications, or just need a diabetes “tune-up,” the program will provide the information and support you need to be successful. Those who are new to diabetes and those who have never received self-management training are especially encouraged to enroll. Family members are always welcome!
